As per Department requirements, parents/guardians have to submit a Return-to-School Medical Declaration prior children returning to school on Monday, 15th of March (3rd-6th)
Please click on the link below and enter our school roll number which is 20139T and click Submit.
Then enter your own name, the name(s) of your child(ren) and the classes they are returning to and then click
Submit Declaration.
Please complete this declaration prior to Monday the 15th of March.
Parents/Guardians must also complete the return to school form if their child is absent from school or is taken from school early any day, whether due to illness or appointment prior to the child coming back into school the next day.
